I think it would be more accurate to separate the two. You may have an orc fetish and a gangbang/freeuse kink. Combined the two make a fantasy, at least semantically. EDIT: Unless you're talking specifically about the action of dreaming of being used by orcs, in which case I think it qualifies as a fetish. Since you could have a fantasy about someone who is forced to dream about said scenarios.
